This short online form is the quickest and simplest way for the majority of our patients to access our clinical team for assistance. Our practice phone lines remain open for anyone with accessibility needs who might find difficulty in completing this form.

Please do not use this online form for urgent or emergency requests. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays.

Booking an Appointment:

Please go online or call the practice on the day ONLY as we will not be able to offer pre-booking GP appointments in advance. This is to ensure that there are appointments available every day to accommodate routine as well as urgent problems.

Patients can see the doctor of their choice. However, when you need to see a doctor urgently it may not always be possible to see your usual doctor.

Call our main switchboard number - 01564 823182 or visit reception in person to book a morning appointment, please call us at 8:30AM and for the afternoon appointment please call us at 1:00PM.

 Home Visits

We ask our patients to visit the surgery if at all possible, as our doctors can see several patients at the practice in the time it takes to make a single home visit. Home visits are only offered to patients who are unable to get to the surgery for medical reasons. Reception staff will take details from patient and a doctor will phone to assess the need for a visit.

 Book online:

To make it easier for you to book and manage your appointments, we’ve implemented a new online booking service.

book your appointment online through the following link : https://patient.emisaccess.co.uk/Account/Login?ReturnUrl=%2f

We make every effort to keep appointment times but there may be occasions when they overrun.

Hollyoaks Medical Practice is also working in conjunction with our local practices to offer Primary Care Services from locality hubs. Additional appointments are available in the evenings, Saturdays and Sundays at various locations throughout Redditch and Bromsgrove.

These appointments might not be with your usual GP or Nurse and some consultations are telephone only. These appointments are pre-bookable in advance - to access these appointments please contact Reception.

Helping the appointment system run smoothly:

If you are unable to keep your appointment please notify the surgery as soon as possible, either by using the online cancellation form (at least 24 hours in advance), or by telephoning reception.

One appointment is for one person. If you want another member of your family to see the doctor or nurse, please book a further appointment.

When arriving at the surgery for your appointment, always inform reception.

Sometimes people can forget to tell the doctor all the important information about their problem. It may help to write their symptoms down.

If you don’t understand what the doctor or nurse says to you, please don’t be afraid to ask for a further explanation.

It can sometimes help to have a partner or friend with you.

If you are seeing the doctor to discuss the result of an investigation or a recent hospital treatment, check with reception first that any important results or letters have been received.
